The objective of KAPP is to improve the self-management skills of those with Chronic Kidney Disease (CKD) stages 2 and 3, including those with diabetes and hypertension. By teaching individuals how to make basic life style changes, we hope to prevent the progression of their CKD.
Each KAPP consists of the following 6 sessions:
Session 1: MEET YOUR KIDNEYS
About Kidneys and their functions
Session 2: CREATE-A-PLATE
Nutrition and portion control.
Session 3: CUT THE FAT
How to reduce the fat in your diet and
reading food lables.
Session 4: STEPPING OUT
Increasing physical activity and integrating
exercise into routine activities of daily living.
Session 5: KNOW YOUR NUMBERS
Learning about proper kidney disease,
diabetes and hypertension self-management
skills to improve glucose control and blood
pressure and to reduce and/or prevent
associated complications.
Session 6: TALK WITH YOUR DOC
Developing strategies in the sharing of information
between doctor and patient and tools to help individuals
succeed in their quest for a healthier lifestyle and
prevent further progression of kidney disease.
Each session will be conducted by health professionals such as physicians, diabetes nurse educators, registered dieticians, and exercise leaders.
