WHAT IS THE PURPOSE OF THE COMMUNITY HEALTH NAVIGATOR PROGRAM?
The Community Health Navigator program will focus on cultivating and empowering community members with skills to provide individualized support to chronic disease patients facing barriers to proper care. Not only will this program help increase patient access to medical services and patient assistance programs, it will help to create a group of Health Navigators who are uniquely positioned to provide culturally-sensitive guidance for patient self-management and adherence to care plans.
What is the target population?
The target population for this program includes those with kidney disease and other related chronic illnesses that are challenged by advanced age, physical disabilities, and socioeconomic limitations.
We are currently recruiting for Community Health Navigators from all over Oahu. The CHNs will undergo a series of training and workshops to ensure that they are equipped with the proper knowledge base to effectively guide and assist the community.
Potential responsibilities of CHNs include but not limited to:
- Help participants schedule health screenings or doctor appointments
- Explain experience with healthcare systems and educate participants on diagnoses and medical procedures
- Help participants find resources for education, transportation or direct patient language interpretation services
- Direct participants to support groups or counseling services
