How much do Health Clinic services cost?
Our services are covered by most medical insurance plans. And, as a nonprofit organization, we offer our services to everyone. Contact us to inquire about your specific circumstances.
 

What can I expect after becoming a Health Clinic patient?
As a patient of the NKFH Health Clinic, you will receive one-on-one care from our Nurse Practitioner and/or Registered Dietitian. We are currently conducting all appointments virtually as telehealth appointments.

We partner closely with you and your other healthcare providers. After each appointment, we will communicate with your primary care physician (PCP) and/or the specialist who referred you to ensure seamless care coordination and collaboration with your healthcare team.
 

How do I become a Health Clinic patient?
Contact us about making an appointment, and ask your primary care physician (PCP) or nephrologist for a referral to the NKFH Health Clinic using the referral form above.
