To address the growing CKD epidemic, we formed the NKFH GFR Alliance which includes the Department of Health, physicians, insurance providers, laboratories, medical facilities, the University of Hawaii, and the pharmaceutical industry. The Alliance successfully implemented several major initiatives. First, automatic GFR reporting, the only statewide GFR reporting in the nation, was instituted for early CKD identification. Second, a treatment algorithm was developed and disseminated throughout the medical community. Third, massive statewide efforts were undertaken to educate the clinicians and to raise public awareness. Fourth, CKD clinics for early chronic disease management and education were established. Fifth, a CKD surveillance system to capture CKD prevalence, incidence, costs, co-morbidity trends, and mortality is being developed.
The National Kidney Foundation of Hawaii GFR Alliance is a collaborative of community stakeholders dedicated to the development and implementation of a statewide CKD prevention initiative.
