Thank you for your interest in donating your car to our foundation. We are able to accept running and non-running cars on Hawaii and will need the completed donation forms and a signed Certificate of Title in our office prior to the completion of the donation. Please fill out the form below.

Primary Donor Information

The primary donor is the person who's Tax ID # will be used when filing an IRS Itemized Tax Form. This person's name must be listed as an owner on the Certificate of Title.

Towing/Pick-Up Service

If your vehicle is being picked up from a service station and parts are missing when tow truck arrives, your car will be rejected. Please ask mechanics/service station personnel to leave your car totally intact.

What hours of the day/night will you permit the tow company to tow your car?

I will file the Notice of Transfer - I understand that the transfer of the vehicle is the date that the car was dropped off at the NKFH office or the date the tow company physically picked up my vehicle, and that the DMV requires that my Notice of Transfer be submitted to their office within 10 days of NKFH taking possession of my vehicle.

I would like NKFH to file my Notice of Transfer and do so authorize them to file it on my behalf. I further understand that they will be mailing my Notice of Transfer via certified mail to the DMV.
